When Paying
by Cash or Check

You may schedule your evaluation online if you plan to pay with cash or check. After you schedule the evaluation, we will send you an e-mail to confirm the appointment. Please note that your appointment is not confirmed until you receive the e-mail and acknowledge its receipt. A payment of $125 is required on the day of your evaluation. The balance of your fees will be due on the day of your procedure. The deposit is fully refundable the same day if you are not a qualified medical candidate. If you are a qualified candidate, the payment is not refundable should you elect not to have your vision corrected after your LASIK evaluation is performed. Schedule an evaluation now
